Health Exams
A student must have a physical health examination within one year prior to entering preschool,
kindergarten, and 6th grade, students new to the school district, regardless
of age or grade, must show proof of exam and immunizations within 30 days of registration.



Immunizations
Students must show proof of basic immunizations and required boosters for Diphtheria, Tetanus, Pertussis, Polio,
Red Measles, Mumps, Rubella, Haemophilus InfluenzaeType B, Varicella  and Hepatitis B.

Lead Screening
Students entering preschool and kindergarten must show proof of lead screening. It is mandatory for students
living in zip codes 61101,61102,61103, and 61104.

Dental Exams
All Illinois children in kindergarten, 2nd and 6th grades are required to have an oral health exam by a
            a licensed dentist prior to May 15th of the school year. If a child fails to present proof of exam,                  
              the school may hold the child's report card until written proof is established or until proof that an exam     
               will take place within 60 days of May 15th deadline.



Eye/Hearing Exams
As of August 2009 the Illinois Department of Public Health (IDPH) will no longer provide vision and hearing
screening free of charge. The cost for each will be $2.50. Below are the grades that are
mandated by the IDPH
to be screened;

Hearing Screen - Kindergarten, 1st grade, 2nd grade, 3rd grade, IEP students, and new students.

Vision Screen - 2nd grade, IEP students, and new students

For children in the other grades, I cannot stress enough how important it is to continue to have them screened for
vision and hearing yearly .  Last year, in the older grades, we found two students requiring glasses.  Had the
parents decide not to screen their child, this would have gone undetected until a problem with their grades were
seen.  If you would like your child screened please fill out the appropriate forms and return them to the office
with the fee.

Students entering Kindergarten still require a eye examination with an ophthalmologist or optometrist.


Waiver
The Department of Public Health shall establish a waiver for children who show and undue burden or lack of access
to obtain either a dental or eye exam.

Objections
Children, whose parents or legal guardians object to health or dental examinations, or to the required
immunizations, may submit a letter explaining their religious beliefs and request an exception to the law.
